Beim Infarkt kommt es infolge einer Koronarthrombose zum (partiellen) Koronarverschluss mit fortschreitendem Zelltod im nicht durchbluteten Myokard. Dank verbesserter Reperfusionsstrategien konnte das \u00dcberleben von Infarktpatienten in den letzten Jahrzehnten deutlich verbessert werden. Patienten mit gro\u00dfem Infarkt haben allerdings weiterhin ein hohes Risiko, einen kardiogenen Schock oder eine chronische Herzinsuffizienz zu entwickeln. F\u00fcr diese Patienten werden neue Therapieans\u00e4tze ben\u00f6tigt.<\/p>\n\n\n\n<p>Ein Herzinfarkt l\u00f6st eine intensive Entz\u00fcndungsreaktion aus. Innerhalb weniger Minuten werden diverse myeloide Zellpopulationen in die Infarktregion rekrutiert. Diese Entz\u00fcndungszellen modulieren den Infarktschaden und koordinieren die Infarktreparatur, indem sie eine Vielzahl von Zytokinen und Wachstumsfaktoren freisetzen. Die Komplexit\u00e4t dieser Signale und deren therapeutische Implikationen werden bislang nur unvollst\u00e4ndig verstanden. Unsere Gruppe hat bioinformatische Sekretomanalysen und funktionelle Screens in myeloiden Zellen durchgef\u00fchrt, um zuvor nicht charakterisierte sezernierte Faktoren zu entdecken. Wir haben so mehrere konventionell (via ER-Golgi System) oder unkonventionell sezernierte Proteine \u200b\u200bidentifiziert, die das Herz nach Infarkt sch\u00fctzen, indem sie den Infarktschaden begrenzen und\/oder Angiogenese und Infarktheilung stimulieren. Diese im Infarktareal freigesetzten Proteine reduzieren so die Narbenbildung und das adverse linksventrikul\u00e4re Remodelling. Wir konnten im Weiteren zeigen, dass eine vor\u00fcbergehende systemische Applikation dieser Proteine in M\u00e4usen die Herzfunktion und das \u00dcberleben nach Herzinfarkt verbessert. Wir haben uns inzwischen mit einem Pharmapartner zusammengeschlossen und untersuchen das therapeutische Potenzial solcher Protein-basierter Strategien bei Infarktpatienten.<\/p>\n\n\n\n<p>Patienten mit gro\u00dfem Infarkt und schwerer Funktionseinschr\u00e4nkung des linken Ventrikels haben ein hohes Risiko einen kardiogenen Schock zu entwickeln. Ein kardiogener Schock f\u00fchrt zu Blutdruckabfall und Minderperfusion und Versagen kritischer Organe und ist mit einer hohen Mortalit\u00e4t assoziiert (bis zu 50% nach 30 Tagen). Derzeit sind keine pharmakologischen Ans\u00e4tze verf\u00fcgbar, um das \u00dcberleben dieser Patienten verbessern. Gro\u00dftiermodelle sind n\u00fctzlich, um die Bedeutung neuer Konzepte zur mechanischen Kreislaufunterst\u00fctzung im kardiogenen Schock zu untersuchen, erlauben jedoch keine genetischen Manipulationen und Arzneimittelscreenings in gr\u00f6\u00dferem Ma\u00dfstab. Wir haben daher unl\u00e4ngst ein erstes Mausmodell des kardiogenen Schocks entwickelt.
